By April 1, 2022, there will be a secured online form on our website, in which patients will be able to provide the necessary information in advance; DAH Best Healthcare Services need to have this information in order to be able to administer the COVID-19 test. Currently, we are asking patients to complete this information when they arrive in person before they are tested.
